Your first visit

Three simple steps

1Book your visit. Choose a time with our online scheduler or call (912) 764-4403. We are open Monday to Friday, 8:00 am to 5:00 pm.
2Fill out your forms. Print and complete your patient forms before your appointment to lower your wait time.
3Bring what you need. Your dental insurance card if you have one and a list of any medications you take.

We accept most insurance plans, and our Financial Coordinator will help you make the most of your benefits.
